Any person with a diagnosed developmental disability 18 years of age and who resides in the Calgary region is eligible for supports. The need for services must be in the rehabilitation and not psychiatric, medical,substance abuse or personality disorder areas. The prospective individual must demonstrate a need and a desire for the types of support services being requested. The applying candidates must not have a past history of violent physical aggressiveness or criminal behaviour. Applications from persons under the age of eighteen (18) are given consideration based upon their unique circumstances and presenting conditions. The Executive Director will take applications from candidates outside of the above entrance criteria to the Board of Directors for their approval. Any person wishing to make application to receive service must fill out an Intake Package (see intake link or email This e-mail address is being protected from spambots. You need JavaScript enabled to view it ) If necessary, an appointment will be arranged with the Client Services Coordinator to assist in the completion of the package. Upon completion, an appointment will be set up with the individual, his/her family and the Client Services Coordinator to discuss the need for service, the available alternative and appropriateness of the vacant program space for the individual. The individual will then be accepted for a 1- 3 month trial period (assessment phase). This will ensure that the program will be able to adequately provide the services required for the individual. At the conclusion of a successful trial period, another meeting will be arranged to formalize ongoing services for the applicant. |
